Allegiance COBRA Services, Inc. was formed on December 12, 2003 and is a sister company of Allegiance Benefit Plan Management, Inc. (ABPMI). ABPMI has been providing COBRA administration since 1989.

FAQ

After you quit, you will receive a COBRA packet that will include the cost of the coverage, forms to fill out to sign up for coverage, and deadlines you must enroll by in order to qualify for COBRA. You need to fill out the forms and mail them to the address shown on the form within 60 days of your notification or 60 days from the date of loss of coverage, whichever is later. The letter accompanying the forms will state the exact date by which you have to sign up.

It depends on the Qualifying Event.


Continuation is available for up to 18 months for loss of coverage due to employment termination, reduction of hours or retirement of the employee, up to 36 months for loss of coverage due to death of the Participant, divorce or legal separation from the Participant or dependents who are no longer eligible dependents as defined by the plan. Up to 29 months if, within 60 days of the Qualifying Event (termination of employment or reduction of hours) the employee or any of the employee’s dependents who are Qualified Beneficiaries are or become disabled as defined under the Social Security Act and provide a copy of the determination of disability letter from the Social Security Administration to the Employer within 60 days of such determination.

Premiums are due the first of every month with a 30 day grace period. We will accept payments that are postmarked within the allowable grace period.
Yes, each covered person has his/her own separate right of election under COBRA, and their premium amount will be the Participant Only or the PQB only rate listed.
Prescription plans are part of a live system that is updated on a weekly basis and are only updated once payment has been received for the month. If payment has been received and prescriptions are still being denied, it could be that eligibility has not been updated yet. In those cases a simple call to us to manually update eligibility will release the prescription. If you pay full price for your prescription you may be eligible for reimbursement. Forms are available upon request.
There are several ways. You can simply not pay the premium - COBRA will be automatically canceled at the end of the last paid month. You may give us a call and let us know you wish to cancel. The preferred method is in writing, either by e-mail or letter. There is also a space on the premium coupons asking if you have become eligible for other coverage that you may circle yes and return to us, and there is also an eligibility questionnaire sent out every 90 days with a similar question.
